(1) Student Course Evaluation as of 2nd Semester A.Y. 2019-2020; and
(2) Midterm Grade Slip of First Summer Term if you have at least 3.5 grade for pre-requisites of the courses you intend to enroll for the Special Summer Term. Otherwise, attach Final Grade Slip of First Summer Term.

(1) Student Course Evaluation as of 2nd Semester A.Y. 2019-2020; and
(2) Latest Final Grade Slip (2nd Semester A.Y. 2019-2020).
